How will the exchanges effect brokers in New Hampshire. I have heard that commission structures are $6-$10 pmpm.

Somewhere in the volumes of PPACA, it says that an employer has to give employees 60 days notice of a material modification in the plan. It also says that this notice is only required for off anniversary changes. Can you point me to where this is written or if Benefit Mall has given out something that states it in writing?

Regarding 2013 and the FSA limit of $2,500 -- your language says "beginning January 1, 2013 ..." but isn't clear if it is plan years beginning that date. Suppose you have a 10/1 plan year and allow $5,000 in an FSA. Do you have to reduce that as of 10/1/12 or 10/1/13? Thanks. Not all FSAs run calendar year.

The question I have is simple. If an employee in a large group declines employer coverage and subsequently enrolls voluntarily through the individual exchange - is the employer fined $2,000 (or whatever the amount is) beginning in 2014?”

A group is currently grandfathered. They have roughly 60 employees, 50 of which are seasonal. Within the state of Colorado that would make the group small group as the state would not count seasonal employees as eligible and/or full time. If the group were to move carriers and become a non-grandfathered plan, come 2014 would they have to provide benefits to those employees would are considered seasonal?

Assuming the MLR remains as is, will carriers in the small group market pool all their claims to determine if a refund is due and then return an equal percentage to each employer or will the MLR apply to each small group employer individually?

Has the 1099 reporting requirement in the Healthcare Reform bill been amended and signed by the President. I know that it was amended in April but I have not heard whether or not Obama has signed the amendment.

Heard from a broker that "private equity control groups" must offer identical medical benefits to all the employees/employers within these groups effective 4/1/2011. Is this accurate? If so, I need direction on finding more in writing.

I have a fully insured client who is owned by a much larger entity in OK with a couple 1000 employees. They bought a company some years ago and it is operated as a completely separate entity. They got a call from the CFO of the larger entity seem to believe the new legislation will require the larger entity to include them in their self-funded plans required by law( PPACA) ..they keep sighting the control group rule in the new legislation ….I have heard this item was abandoned …do you have an resource we could get some timely information on this subject?

I am trying to get a better understanding of the wellness provisions under PPACA particularly the Wellness Grants the feds have supposedly been issuing. I know the high level qualifications, but do not know the specifics of how a group applies. Can you point me in the right direction?

I recently read that on December 29th, 2010 the President signed HR 6517, section 116 of which extends the maximum COBRA coverage period for Heath Tax Credit recipients from December 31st, 2010 to February 12th, 2011 and also extends the subsidy. I need additional info on this. Is this only for existing COBRA participants that were previously qualified or does this open the window for new participants as well?
